Information you choose to share
The contact form asks for your name, email address, optional organization, area of interest, and message. The careers form asks for your name, email, country, role applied for, experience, availability, optional portfolio link, introduction, and CV.
How form submissions work
These forms send your information to FormSubmit (formsubmit.co), a third-party form processing service, which forwards it to MyHealthSnap’s inbox at myhealthsnap@outlook.com. FormSubmit may use spam protection and process technical information to deliver submissions. Review FormSubmit’s privacy policy before submitting.
How we use information
Contact details are used to respond to enquiries. Recruitment information is used to review applications. Do not send sensitive medical information through either form.
Storage and your choices
Submissions may remain in the receiving email inbox and in the form service’s records. FormSubmit documents a 30-day submission archive; attachments are not included in that archive. You may request access, correction, or deletion of information you submitted by emailing myhealthsnap@outlook.com. Applicable legal requirements may affect what can be deleted.
Website hosting and cookies
The hosting provider may process standard request information needed to serve and protect the site. This website does not add advertising trackers or analytics cookies. Third-party form verification may apply its own technologies when you submit a form.
